Position Summary
A Population Health Coordinator is a key member of the FullWell Quality team that will help outreach to patients and coordinate with their primary care provider so that their outstanding care needs get addressed. This role requires a strong communicator who can speak to patients to encourage/schedule preventive screenings, coordinate with primary care practices to schedule visits, educate patients on their care needs, and partner with patients' primary care provider to develop an appropriate action plan. We are looking for someone with initiative who sees it as their mission to try to encourage as many patients as possible to address outstanding care needs like cancer screenings, diabetes exams, and other recommended actions. This role will be based at FullWell's office in Englewood, CO but may involve travel to different primary care practices in order to support them best. You will report to the Director of Quality and be part of a fun, growing company that is trying to approach healthcare differently and make a difference in Colorado.
Minimum Education Requirements
:High School Diploma or GED
:MA, LPN, or relevant healthcare experience
Minimum Experience Requirements
:2:3 years of prior healthcare experience in an outpatient medical practice preferred
:Proficient computer skills : report generation, data entry
:Patient communication
:Outpatient medical practice
License and Certification
:MA, LPN or other relevant healthcare license or certification
Position Duties (essential functions denoted with an *)
:Verbalizes an understanding of the value of measurement and the importance of continuous quality improvement data*
:Uses quality measures to improve performance and accountability for patient outcomes, patient experiences, and safe delivery of care*
:Manually conducts reviews of patients' medical records seeking evidence/supporting documentation of care gap completion*
:Through internal payer attestation processes, conducts manual care gap closures for records that have been completed and adequately documented*
:Partners with primary care practices to provide timely and accurate lists that reflect outstanding gaps in care for the practice's patient populations*
:Outreaches to patients via phone, letter, and possibly email, to close gaps in care like preventive cancer screenings and diabetes exams*
:Conducts follow up outreach to ensure patient action was taken*
:Coordinates with primary practices, ensuring that the patient outreach performed is approved and directed by the primary care provider*
:Documents ongoing status, interventions, patient responses, and outcomes in an accurate and timely manner*
:Appropriately escalates certain patients to an appropriate clinician if you believe they are high risk and may benefit from more intensive support*
:Collects, tracks, reports, and communicates data and data analyses as required*
:Uses information technology to communicate, man
