A Care Navigator is a key member of the FullWell Care Management team that will support the RN care manager(s) and help outreach to patients following ED visits, in:patient hospitalization, and stable disease management patients. This role will coordinate with the patient's primary care provider for medication refills, post: hospitalization appointments, and any RN delegated duties, etc. This role requires a strong telephonic communicator who can speak to patients to encourage/schedule preventive screenings, coordinate with primary care practices to schedule visits, assist patients with their care needs, and partner with their assigned RN care manager. We are looking for someone with initiative who works well with our Care Management team to ensure excellent quality patient care with great outcomes. This role will be based at FullWell's office in Englewood. This position will report to the Regional Care Manager for the Care Management team and be part of a fun, growing company that is trying to approach healthcare differently and make a difference in Colorado.
Minimum Education Requirements
- Diploma from an accredited Medical Assistant program
Minimum Experience Requirements
:2:3 years of clinical healthcare experience in an outpatient medical practice
:Proficient computer skills : report generation, data entry
:Computer data entry
:Clinical judgement skills
License and Certification
:Certification as a Medical Assistant, preferred
:Current LPN license for applicable state of employment (if applicable)
:Promote patient education, self:management support and timely delivery of health care.
:Identify and work with individual and patient populations as required for clinical or medical reasons.
:Outreach to patients via phone, letter, and possibly email
:Coordinate patient care with primary practices
:Communicates patient needs and arranges services with appropriate community resources
:Document ongoing status, interventions, patient response and outcomes in accurate, timely manner
:Appropriately escalate certain patients to an appropriate clinician (RN) if you believe they are high risk and may benefit from more intensive support
:Collects, tracks, and reports data as required
:Use information technology to communicate, manage knowledge, mitigate error, and support decision:making
:Use current healthcare research findings and other evidence to expand clinical knowledge, enhance role performance, and increase knowledge of professional issues.
:Develop working relationship with supported primary care practices
:Takes action to achieve goals identified during the evaluation process.
:Demonstrates a commitment to the organization through ongoing participation in team: based meetings.
:Builds trusting relationships with internal and external customers, i.e. physicians, nurse practitioners, PAs
