Schedule: Full Time
Administer care management and coordination activities in the outpatient setting by working with Patient Centered
Medical Home Teams, clinic Medical Director, clinic administrators, and outside entities to coordinate patient care,
improve quality of medical care and patient safety. Assumes responsibility and accountability for facilitating,
communicating and collaborating with the healthcare team and the patient/ family, utilizes best practice to maximize
patient engagement and patient self:management efforts. Demonstrate personal accountability for relationship:based
care, organizational mission and core values.
Minimum Education Requirements
:High School Diploma or GED
:MA, LPN, or relevant healthcare experience
Minimum Experience Requirements
:Minimum 3 years experience in an outpatient medical practice
:Proficient computer skills : data entry, retrieval and report generation
:Utilize the Institute for Healthcare Improvement Chronic Care Model as foundation and framework for chronic illness
:MA, LPN or other relevant healthcare license or certification
:Certification as a Certified Medical Assistant or Certified Clinical Medical Assistant Required within 90 days of hire (if applicable)
:Diploma from an accredited medical assistants program and externship completed is preferred (if applicable)
:Current Colorado nursing license/ Current nursing license from a participating state in National Licensure Compact within 90 days of hire or transfer into position (if applicable)
Position Duties (Essential Functions)
QUALITY OUTCOMES/QUALITY IMPROVEMENT/COMMUNITY OUTREACH : 40
Works with providers and patients to ensure chronic disease metrics are met for each patient and patient population. Reviews
patient interim health encounters and problem solves challenging areas. Verbalizes an understanding of the value of measurement and the importance of continuous quality improvement data. Use quality measures to improve performance and accountability for patient outcomes, patient experiences and safe delivery of care. Assists with coordination of clinic:based Quality improvement, evidence:based practice and research activities. Use current healthcare research findings and other evidence to expand clinical knowledge, enhance role performance, and increase knowledge of professional issues. Develops working relationships with community resources for the benefit of patients. Participates and coordinates community outreach efforts on behalf of the clinic to improve community health.
