I. Position Summary:
Under the direct supervision of the Director of Pharmacy/Pharmacy Clinical Coordinator, and day to day direction of the ED Pharmacist, performs functions related to Medication Reconciliation for all admitted patients. These activities may include but are not limited to: daily interaction with admitted patients and family members to gather home medication information, contact outpatient pharmacies and other healthcare facilities to gather patient-specific medication information, complete phone calls to pre-op patients prior to scheduled surgery, documentation of all interactions in electronic medical record and pharmacy tracking system, work closely with overseeing pharmacist to identify interventions, collaborate with other Medication Reconciliation technician/s to capture all patient admissions.
In addition to the duties outlined above, the individual may be asked to perform functions related to pharmaceutical preparation and distribution. These activities may include but are not limited to: fills unit dose cassettes, enters and fills unit and physician orders, produces labels and prepares IV admixtures, charges and reconciles billing documents audits supplies in nursing units, prepares medications, enters medication orders in computer. May stock and distribute pharmacy supplies. May be responsible for ordering, controlling, dispensing, preparing, delivering, filing documents, and destroying controlled substances within the facility. This position may make routine visits to off-site locations ( Free standing EDs, etc) for medication distribution/management and inspection
